| CREDIT CARD AUTHORIZATION |
When an invoice is going to be paid with a credit card not issued to the company but in the name of an individual of the company or when an international credit card is used, we need to obtain the individuals authorization. That individual will need to complete one of the two forms below.
One-Time Credit Card Authorization Form
Blanket Credit Card Authorization Form
If a transfer of funds is requested from an approved credit card account and that credit card account contains insufficient funds to cover the transfer, the Cardholder will be charged $50.00, which will be charged to the Cardholder's Purespeedplus account and will show up as such on the invoice.

| This credit application can be used by businesses seeking billing privileges with Purespeedplus. ... Click here to print a credit application. All accounts are COD until a credit application has been completed and/or a deposit has been received. The credit application must be reviewed, and approved by the Purespeedplus. If client fails to pass a credit check, a Letter of Credit and/or an
increased deposit may be required. The Accounting Director will make the
final determination of the financial requirements. |
